We welcome you as a patient to our practice and hope you will find this website helpful. It is intended to inform you of the services available and to help you get full benefit from them. As a practice, we wish to help you recover as rapidly as possible from any illness and help you avoid ill health by encouraging a healthy lifestyle.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available at the surgery if you need one.

Medication requests (48hr processing time)
Fit note requests (can be back-dated, but not future-dated even over a weekend/bank holiday)
Routine queries

Once submitted, it will be reviewed by the practice.

Medication requests will be actioned, and we will only contact you if there is an issue. It will otherwise be sent to your nominated pharmacy.
You will be contacted once your fit note has been actioned.
For routine queries, you may be signposted to alternative services, receive a message from reception, or be offered an appointment.

You do not need to contact the surgery in any instance unless your symptoms change or worsen.
