Orchid House Surgery now offers routine week day early morning appointments between 7.00am and 8.00am and evening appointments between 6.30pm and 8.00pm.We also offer one Saturday morning a month between 8.30 and 10.30 am for those patients that are unable to attend our normal opening hours between 8.30 am and 6.30 pm, Monday to Friday, due to daytime commitments

The phone lines will not be manned during this time, nor can prescriptions be collected or results given. These appointments are available to any patient unable to attend a routine appointment during normal surgery times.
PLEASE SPEAK TO THE RECEPTIONIST IF YOU NEED AN EVENING OR SATURDAY MORNING APPOINTMENT.
DONT FORGET THE FLU / PNEUMONIA SEASON STARTS IN OCTOBER EVERY YEAR. PLEASE RING TO RESERVE YOUR FLU OR PNEUMOVAX INJECTION FROM SEPTEMBER ONWARDS.
Summary Care Record
You should have received a letter from the NHS advising you, that unless you opt out you will have a summary care record set up.This will enable certain details of your records, such as what medications you are taking, what medications you have reacted to in the past and if you have any allergies, and what conditions you have that might mean you shouldn’t have certain medications, to be accessed by people involved in your immediate medical care.
We currently have a Care Record table in our waiting room, please see this for more information.Alternatively you can telephone the NHS Care Records Service Information Line on 0845 603 8510.
If you do not wish to have a Summary Care Record please make sure you opt out as by not responding you are assumed to have agreed.
You can collect an opt out form from our dedicated care record table (in the waiting room), write a letter informing us of your decision to opt out or visit www.dorset-pct.nhs.uk/health_services/electronic_health_services/summary_care_record where you can download a copy of the opt out form and drop it in to us.
If you decide you would like a Summary Care Record then you do not have to do anything, you will automatically be loaded onto the shared system.
Please see our care record table for information on how to view your care record once it has been uploaded.
Please remember it is completely your decision whether you choose to allow your records to be loaded onto the national database or to opt out, signing the necessary paperwork.

The GP Patient Survey
Every three months the NHS sends out questionnaires to a random sample of patients from all practices, so that improvements, where possible, can be made. Feed back on these questionnaires has a direct impact on the funding of GP surgeries and the services they provide and it is therefore very important that they are filled in, but filled in carefully, as some questions can be a little confusing.
If you receive a questionnaire, please read it carefully and try to return it, as low numbers of completed surveys will have an unbiased affect, possibly leading to reduced funding.
